Understanding Testosterone Levels in Men: Norms and Testing

Testosterone, a hormone primarily associated with male reproductive function and libido, also plays a significant role in the development of bone and muscle tissue. It is produced by the Leydig cells in the testicles, and its levels can vary based on factors such as age, physical activity, lifestyle, and medications.

To assess testosterone levels, it is important to understand the distinction between “total” and “free” testosterone. Total testosterone includes both free testosterone (not bound to blood proteins) and sex hormone-binding hormone. The specific test recommended for an individual depends on their symptoms and the results of their examination, as determined by a urologist or andrologist.

The normal range of testosterone in men can vary slightly across different laboratories. To monitor the hormone’s dynamics, it is advisable to undergo tests at the same laboratory consistently. Here are the approximate normal ranges for total testosterone based on age:

  • Boys aged 15-16: 5-29.2 nmol/l
  • Men aged 16-21: 4.10-32.92 nmol/l
  • Men aged 21-50: 5.72-26.14 nmol/l
  • Men over 50: 3-27.35 nmol/l

Free Testosterone

For free testosterone, the normal range in men is approximately:

  • Boys aged 13-18: 1.4-48.2 ng/ml
  • Men aged 19 and above: 15-50 ng/ml

When should you consider taking a testosterone test? Testing should be done when there are specific complaints or indications, and it should be initiated by a specialist such as a urologist or an andrologist. These experts will assess the results of the examination and prescribe treatment if necessary.

Total testosterone level testing may be recommended in cases of:

  • Decreased libido
  • Difficulties in conceiving a child (when male infertility is suspected)
  • Erectile dysfunction
  • Testicular neoplasms
  • Hypothalamic or pituitary gland disorders

Free testosterone analysis is typically recommended for:

  • Erectile dysfunction
  • Suspected onset of andropause (male menopause)
  • Chronic inflammation of the prostate gland
  • Osteoporosis, among other conditions
