Tilfeller med oocytter og embryoer av d\u00e5rlig kvalitet anses som spesielt vanskelige \u00e5 f\u00e5 bukt med. I tillegg kan verken leger eller pasienter p\u00e5 forh\u00e5nd identifisere og forebygge en rekke problemer som kan oppst\u00e5 under behandlingen.<\/p>\n\n<h2 class=\"wp-block-heading\">Hva er kjernefysisk overf\u00f8ring?<\/h2>\n\n<p class=\"wp-block-paragraph\">Kjerneoverf\u00f8ring (NT) er en ny metode for behandling av infertilitet, som g\u00e5r ut p\u00e5 \u00e5 overf\u00f8re pasientens kjernemateriale til cytoplasmaet i et donoregg. Logikken bak denne metoden er at visse negative faktorer kan v\u00e6re knyttet til eggcellens cytoplasma og kan overvinnes ved \u00e5 bytte ut cytoplasmaet.<\/p>\n\n<p class=\"wp-block-paragraph\">Interne embryonale faktorer som f\u00f8rer til embryostopp:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>Funksjoner av genuttrykk<\/li>\n\n\n\n<li>Forstyrrelser i mitokondrielt DNAs mengde og funksjon<\/li>\n\n\n\n<li>Funksjoner ved metylering<\/li>\n\n\n\n<li>Sm\u00e5 ikke-kodende RNA<\/li>\n\n\n\n<li>Kromosomavvik<\/li>\n\n\n\n<li>Metabolsk profil av embryoet<\/li>\n\n\n\n<li>Spesifikke morfologiske kjennetegn, s\u00e6rlig vakuolisering<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\">Mitokondrienes rolle i eggcellene<\/h2>\n\n<p class=\"wp-block-paragraph\">Oocyttene har det st\u00f8rste antallet mitokondrier og kopier av mitokondrielt DNA (mtDNA) sammenlignet med andre celler i kroppen. Mitokondriene i oocyttene har to hovedoppgaver: \u00e5 st\u00f8tte oocyttens metabolske behov og regulere apoptose, samtidig som de fungerer som et reservoar av intakt mtDNA for fremtidig utvikling av avkommet. Under follikkelrekrutteringen kan mitokondriene \u00f8ke fra 6000 kopier av mtDNA til 200000, noe som gj\u00f8r dem mer s\u00e5rbare for mutasjoner og delesjoner.<\/p>\n\n<h2 class=\"wp-block-heading\">Metoder for kjernefysisk overf\u00f8ring<\/h2>\n\n<p class=\"wp-block-paragraph\">Det finnes flere metoder for kjerneoverf\u00f8ring som brukes b\u00e5de f\u00f8r og etter befruktning:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>F\u00f8r befruktning: Overf\u00f8ring av kimbl\u00e6rer (GVT), overf\u00f8ring av kromosomdelingsspindler (MIST | MIIST), overf\u00f8ring av f\u00f8rste pollegeme (PB1GT)<\/li>\n\n\n\n<li>Etter befruktning: Overf\u00f8ring av det andre polare legemet (PB2GT), Pronukle\u00e6r overf\u00f8ring (PNT)<\/li>\n<\/ul>\n\n<h2 class=\"wp-block-heading\">Erfaring fra IVMED-klinikken<\/h2>\n\n<p class=\"wp-block-paragraph\">Blant pasientene som gjennomgikk fisjonspindel- og polkroppsoverf\u00f8ring (MS og PB1), er gjennomsnittsalderen 40,3 \u00e5r, og gjennomsnittlig antall tidligere IVF-ICSI-fors\u00f8k er 3,3. Vi brukte 99 M II-ocytter fra pasienter og 166 M II-ocytter fra donorer. Vellykket rekonstruksjon ble oppn\u00e5dd i 86 % av tilfellene, med en fragmenteringsrate p\u00e5 60 % og en blastocystdannelsesrate p\u00e5 29 %.<\/p>\n\n<p class=\"wp-block-paragraph\">Pasientene som gjennomgikk pronukle\u00e6r overf\u00f8ring (PNT), hadde en gjennomsnittsalder p\u00e5 39,7 \u00e5r, og gjennomsnittlig antall tidligere ICSI-fors\u00f8k var 3,8. Vi brukte 185 M II-ococytter fra pasienter og 277 M II-ocytter fra donorer. Vellykket rekonstruksjon ble oppn\u00e5dd i 83,4 % av tilfellene, frekvensen av stamcellefragmentering var 72,2 %, og frekvensen av blastocystdannelse var 44 %.<\/p>\n\n<h2 class=\"wp-block-heading\">Konklusjoner<\/h2>\n\n<p class=\"wp-block-paragraph\">Oocyttrekonstruksjon kan i visse tilfeller vurderes som et alternativ til oocyttdonasjon. Valget av en spesifikk teknikk avhenger av antall og kvalitet p\u00e5 oocyttene, samt den kliniske situasjonen. Mitokondriesykdommer er ikke alltid en indikasjon for kjerneoverf\u00f8ring hvis det ikke finnes teknologi for \u00e5 fjerne maternell mtDNA fullstendig. NP-er er mindre effektive for eldre kvinner. Hvert tilfelle er unikt og krever en individuell tiln\u00e6rming.
